GEN: Need bookplates I can put thru a laser printer

I'm trying to design a book plate for our reading program (kids get
to pick a new library book before it is put on the shelf, get their
name in it and be the first to take it out.)  Does anyone know where
I can get blank plates with adhesive backs that could go thru a laser
printer?  I thought I saw this somewhere, of course can't find it
now.  Or did I make this whole thing up?
