greetings,

i am interested in finding information about the current procedures
publishers use to alert librarians, and other interested folks, of
errors or omissions in texts.  in your experience:

1.  generally speaking, how often do materials need to be updated due
to errors/omissions (often? never?)
2.  what are the types of materials that most often need addenda
(reference, textbooks, general fiction, etc)
3.  how are you alerted when an update is added
4.  what are the updates called?  addenda? errata? supplements?
5.  are these usually available online or do you usually receive them
in print
6.  is there any resources available on this topic, i've done some
general searching on the web but have only come across scientific or
computer related items.

any information would be much appreciated!  please feel free to reply
to me off the list and thank you in advance.
