Observation/Evaluation Form

Hello All,

I'm looking for samples of teacher observation forms that have been modified
to fit an evaluation of a library media specialist.  If anyone has such a
form that your principal uses to evaluate your teaching performance, I would
greatly appreciate a copy.  Please attach and email or fax to me at
