TARGET: permission form

Does anyone have a standard release form for students that gives =
permission for their image to be shown within the school community?  =
This would cover classroom tapes of oral presentations, videos of school =
TV show, videos of school plays, etc.

Thanks so much.  We would like to have the form signed by parents at the =
beginning of the school year and then kept on file. =20
