TARGET>Author Visit Evaluations

This year I was fortunate enough to receive a grant to invite an author to
visit my k-12 school. Now it's time to complete my end-of-project report
and I need to include some evaluations. Since I am a firm believer in NOT
reinventing the wheel, I would appreciate your ideas for an evaluation.
Some students have spontaneously given me evaluations through their
comments, and I've asked them to write me a letter saying the same thing
on paper, but I need more concrete ideas. I hope I'm making my request
clear...any ideas or rubrics or questionnaires, etc. you would share would
be most appreciated.
