Target: Evaluation

Hi,
    Are any of you familiar with a tool for evaluating a media center
program that can be completed by teachers?  I am also looking for a survey
that my volunteers can complete to have them help me evaluate the program
and how I can make their job as volunteers easier.  Can anyone direct me?
