Statistics and evaluation

Hello,
I am a school librarian in the South of Sweden. Right now I am planning
for the new semester in my school library. This year I would like to
have a good form for statistics. Can you help me with this? I would like
to have statistics about what we do in our school library. Circulation
of course, but most of all the activites that are so difficult to
measure. Classes visiting, project taught, collaborative plannin and so
on. I would be greatful if you could send me some sample of such a form
to use every day. Thank you!
