TARGET: library volunteers

Does anyone have a library volunteer questionnaire form / sign-up sheet that
they could send me?  I'd like to recruit some part-time parent or
grandparent volunteers, and I'd like to send a note home with the students
the first week of school...which is right around the corner.  I thought
about including on the form some specific tasks that parents could volunteer
for, and maybe even some specific time slots. Any suggestions?
BTW, I've checked the Archives, and found many great ideas concerning
volunteerism in the library, but haven't come across any forms yet. Also,if
anyone has volunteers' duties checklists or training manuals that you could
email me, I would GREATLY appreciate it!
