Technology Survey

I have just been appointed the Technology Facilitator in my K-4 building and
it will be my responsibility to set up a technology staff development plan
for the staff.  Before I can do that I need to do some kind of a survey to
find out where people are in their use of technology in the classroom.

Can someone point me in the direction of a survey that's been used and found
effective for this kind of thing?  I need to know things like how
comfortable they are with the hardware, their use of software, what they
think they need, etc.
