GEN Writing adventure stories

A fellow librarian is looking for a one page instruction sheet on how to
write an adventure story. She had a copy quite some time ago and isn't sure
if she received it in library school or somewhere else.  It basically was a
flowchart showing the steps of an adventure story.

Please let me know if this sounds familiar and where I could obtain a copy.
