According to Strunk & White, titles should be italicized, or underlined in 'manuscripts'. Since underlining "stands for" italics (due to the fact that older typewriters could not italicize), you should use one style or the other, but not both in the same work. In reports and research papers, titles of books are shown with underlining in In-Text Citations and bibliographies, and I would expect them to also use underlining in other references to books in their papers, as that's far less confusing than changing styles.
