I just started using Mackin, an on-line company (at http://www.mackin.com/), this year and I'm sold! Their prices are good, the bindings are great, they do free order typing, cataloging, processing and shipping (for orders over $50), and will do Kapco covers on any PB title for 79 cents extra each. Their customer service is good and turn-around time on most titles is great. They can't provide everything (certain publishers are only available directly) but so far, they've had almost everything I've needed, even the Christian titles that are often hard to find from vendors. I used to use Follett, but the paperwork was a nightmare. I've also used Storyhouse (out of business now), Turtleback (Demco) and BTSB, but I like Mackin best, since the books are on the shelves now in days instead of weeks!
