HIT-->Evaluation Form for Subscription Databases

I didn't receive any forms in response to my request for methods of
evaluating online subscription databases, but I had several requests for a
hit. So, I've tried to design the beginnings of a form on my own in hopes
that you all will let me know how I can improve it.

I've posted links to the form in three different formats (Microsoft Word,
Rich Text Format and PDF) at http://www.debwaugh.com/standards.htm.
