California has a law or it's in the Ed Code that parents are respsonsible for replacing damaged/lost books. I have a copy at school, but not at home so I can't tell you the specific working. This year, I had every student turn in a form before checking out books and it was a huge hassle. Net year, I'm only going to have the 1st graders and students who are new to the school fill out the form. The form is more for parent notification of the consequences of damaging/losing a book. The library is an intregral part of a student's education. Some of our primary classes have home reading programs where they students must take a library book home and read to someone every night for a few minutes.
