ELEM: database evaluation for students

Good afternoon,

I have had my fifth grade students researching a topic of their choice
using our three subscription databases. (Ebsco's Searchasaurus, eLIbrary
Elememtary, and Infotrac Jr)

I had them choose three topics they were interested in and look up each
in all three databases.  They then choose a final topic and found a
magazine article to read and I allowed them to print one picture
relating to their topic.

I would like them complete some form of evaluation of the dtatbases they
worked with, but most of the evaulation forms I've found are too
difficult, and I'm having a hard time creating what I want.  Do any of
you have an evaluation form that you have successfully used with
elementary students that you would be willing to share?  If you would
like, I would be willing to share the materials I created for this
project.
