TAR: Student User Surveys

Hi,
   Does anyone have any student user surveys that they've used  either at the
elementary, middle, or high school levels? Our department needs to  send
these out to the various levels by November 12 and we are searching  for some
survey questions and ideas that have already worked well. We have  already
consulted the new Power Tools. Thanks for any help! Please reply to
