[LM_NET] ELEM & MID: Student Self-Evaluations

Good Morning,
We do not have a space on the report cards for a library grade, which is
fine because it is a collaborative teaching model anyway.  BUT, I do want
students to self-evaluate library skills, behavior, habits, and work
products to have a sense of growth.  Teachers could also share these
evaluations at conferences.  We keep portfolios, so part of the evaluation
would be looking at the work in their folders.  I could create these
evaluations myself, but I'm sure some wonderful LM_NETTERS have great
examples that I could either modify or use.  Is anyone willing to share with
me?  I'd hate to reinvent the wheel if there were some examples out there.
I always receive helpful suggestions and ideas from you all.
