[LM_NET] Book evaluation

Dear all,  I am looking for a good book evaluation form for librarians.
I belong to a school committee that must evaluate books that are brought
up as questionable, our members are all over the world, so we need one
form to use to evaluate fiction and nonfiction books.  Any ideas?
