Hi everyone- as a new library media specialist, I want to be able to document and monitor the services that I provide to the teachers in my school. I have been asked by my supervisor to create a questionnaire/survey that I would give to teachers after I provide classroom instruction so that I would be able to collect tangible feedback from them. I think this is a good idea for me. I've attended many workshops and conferences over the years where participants have been given surveys at the end of the program to collect meaningful about it. Has anyone done this in their lmc? I would be interested to see your questionnaire and/or to hear about your experiences with evaluating your lmc services. Thanks. Andrew J.
