[LM_NET] GEN: challenged book form

LM_Netters...

I have been working on updating our Library Policy and Procedures manual.  In this 
process I found that we have no form available for patrons who wish to challenge a 
book in our collection.  I would like to have a form in place prior to the event 
actually happening (with hopes that I never have to use it:).  

I have searched the archives and also the ALA website.  I have found a few 
suggestions of things to include, however no actual form.  Would someone who has a 
form such as this in place be willing to send me a copy?  This will help me as I 
attempt to correct this oversight in our manual.
