[LM_NET] how-to mylar book cover

I know this is silly, but I am a new LMS and need help with using the 
mylar book covers.  I can't figure out how to do it for the life of me, 
and I am beginning to feel a little dumb.  

Is there some type of diagram somewhere that gives instructions?

Please help.
