[LM_NET] reading questionnaire

Hello All,

I hope  you can help me with following request I have received from a
teacher in my building.  Please reply directly to me @
mclark2@phoenix.k12.ny.us  Teacher's request follows....

I need some help.

I am looking for a questionnaire that a student would complete to
demonstrate how interested that student is about reading.  It would ask
questions like: how often, how they feel about..., do they like
to...etc.
