As it goes sometimes, I received more replies for a hit than I received surveys.
Here is what I have.

We would like to improve our service to you next year. So…we’d like to know how we
did in 2003-2004. The survey is on the front and back of this page. Please take a
minute to complete this survey for us so we can be better at serving your needs
next year. Return it to your name here’s box by April 30. Thanks!!!.

Please check the appropriate block on the number scale for each statement.

Create grade lelvel documents, telling about things they are working on in their
curriculum, and how the library and my expertise can really bring some pop into
that activity. Come up with activities to tell the teachers you are ready to do
with their curriculum. Make those activities fantastic and word of mouth will
spread the activities.

In order to support what you are doing (or plan to do) in your classroom, I would
appreciate if you could complete this information sheet. We are better able to
locate appropriate resources if we have advance notice.

KISS strategy

3 questions, to go with this which at this time I have misplaced

The original request follows:

I would like copies of any surveys you have given your staff to find out how
you can better meet their needs, both with resources and collaborative
lessons. If you have not sent out a survey, but would like to suggest a
question I could include in the survey I am creating, that would be great
also.

The situation as it is, is that kindergarten and first grade come in on average
once a week and classes in other grades come in maybe once a month. This is
flexible scheduling, and the scheduling part is greatly lacking.

Thanks,

Amy Hailey
