Portland Public Schools has their evaluaton forms online (pdf)

One form fits all - elementary through high school.

North Carolina has one - go to the state educational website and there are
links to the media/technology webpages. You can email the director, Frances
Bradburn, if there are not forms online.

Our district uses our state model (Tennessee) which is old, needs to be
revised, but still being used. You can download the model from our system
website. You have to click through a few pages to get there, but here is how
you locate it:

2. Click on the Administration button on the left side of the SCS homepage. 
3. then - Click on Human Resources (white part of page)
4. Under "Important Links" click on Framework for Evaluation and
Professional Growth link. 
5. When this page comes up, look on the left hand side in the blue border
and click on the Media Specialist Manual
6. It will download in PDF format

Appraisal Instrument (MCPAI-R) at the following link:

This evaluation form was intentionally designed to be appropriate for
media coordinators K-12, whether they have fixed or flex schedules,
whether there is a tech facilitator in the building or not, etc.

REPORTS TO: Superintendent 
FUNCTIONS: Provides leadership and direction for the district's library
media program. 
COORDINATES WITH: Principals, certified library media staff and other
district professionals as directed. 
GENERAL JOB DESCRIPTION: District Library Media Director has the
responsibility of district wide coordination of the library media centers,
including planning, evaluating and reporting. The district library media
director's responsibilities can be categorized as organizational,
instructional development, library media staff development and
communication. 
ORGANIZATION: 
Assesses needs and develops a long-range plan 
Develops policies and procedures 
Recommends appropriate staff, facilities, materials and equipment 
Implements appropriate centralized services 
Provides technical, consultative and managerial support 
Coordinates the district and building library media programs 
Assists with planning construction and renovation of facilities 
Evaluates programs and makes recommendations for improvement 
INSTRUCTIONAL DEVELOPMENT: 
Serves as a member of the district's curriculum development team 
Serves as a consultant in selection and purchase of media and equipment. 
Promotes effective use of library media programs 
Directs evaluation, selection and use of new technology 
LIBRARY MEDIA STAFF DEVELOPMENT 
Orients new personnel 
Fosters the use of new developments and innovations in instructional media
and technology 
Provides in-service education for staff 
Promotes and facilitates communication among library media staff 
Assists with recruitment, hiring and deployment of library media staff 
Provides direction and prioritization when implementing objectives 
Participates in the evaluation of library media staff 
FISCAL MANAGEMENT 
Determines resources needed to achieve library media program goals and
objectives 
Develops and justifies the library media program budget 
Assists building staff in developing building level library program budgets 
Oversees the expenditure of all library media and technology program funds 
Coordinates federal, state and private projects and grants providing funds
for school library media and technology programs 
Acts as a liaison between district financial accounting office and building
media specialist in monitoring expenditures and reconciling account
expenditures 
Assists in preparing expenditure reports as required by the state of WI. And
using WESSA guidelines. 
COMMUNICATION: 
Interprets and promotes the library media program to the school and
community 
Provides official reports and statistics pertaining to library media and
instructional technology services, as needed by the district, DPI, and other
agencies. 
Represents the district on matters relating to media technology 
Maintains liaison with other schools, libraries and community and state
agencies when appropriate 
PROFESSIONAL: 
Assumes an active role in professional organizations and activities 
Continues to acquire knowledge through in-service, workshops and academic
courses. 
Maintains an effective relationship with students, staff, and the community 
Networks with other library media professionals 
TERMS OF EMPLOYMENT: 
Regular school contract, plus one week before and one week after the regular
school year. 
EVALUATION: 
Performance of this job will be evaluated by the principal (s) in accordance
with provisions of the Board's policy on teacher evaluations. 
BUILDING LEVEL MEDIA SPECIALIST 
INSTRUCTIONAL: 
Provides students and faculty with media services and resources 
Creates an atmosphere conducive to learning 
Builds a collection of materials appropriate to the educational program and
student needs 
Provides instruction in the use of library media services and resources 
Assumes an active role in collaborative teaching and curriculum development 
Works closely with faculty to promote effective use of service and resources

Develops and coordinates a program of information literacy skills for
students 
Complies with section 121.02(0) WI. Stats. In selection of materials,
particularly those which "reflect the cultural diversity and pluralistic
nature of American society."
