[LM_NET] Teacher evaluation of lesson

You did such an amazing job last time I am back with another request. Does anyone 
have a form that they give to the teacher after teaching a library lesson or do you 
just sit down with the teacher and discuss the pros and cons of the lesson? If 
anyone has something like this please email it to me. Much thanks. By the way if 
anyone else wants the attachments I couldn't send containing the collaboration 
templates please email me with your email address so I can send them to you
