[LM_NET] TECH TARGET: Creating Forms on websites

I need to create a selection form for our library website that will allow students 
to make choices for summer reading by using a drop-down box that contains the list 
of book choices.  After making their top five choices for books, I then want those 
choices to be sent to an email address.

The format we used last year is no longer available to us, so I need to create 
something new.  I don't want to spend a LOT of time doing this, so I'm curious as 
to what others might be doing with online forms that contain drop down boxes (or 
selection choices / radio button).

If you have ideas, suggestions, or know of a free "website form" creator, please 
let me know.  I need to create something by next week.
