[LM_NET] TARGET: Library Helper Application

Dear Netters:

I am starting my second year as an elementary librarian.  Last year I enlisted some 
3rd and 4th grade friends to be library helpers and things were great but somewhat 
disorganized.

This year, I would like to have a library helper application form available for 
interested students to fill out.  The main purpose of this form would be to ensure 
that they are responsible and willing to dedicate some of the pre-school morning 
time to helping.  In addition, I want them to turn this form into their teacher, so 
that the teacher can make additional comments as to if they think this child would 
be able to perform the library duties.  Does anyone have such a form or could you 
at least recommend some questions or sections that should be on this form?
