First and foremost, thanks to all who contributed information to this hit. I had an overwhelming response of ideas and several application samples. So, here is the information I received...enjoy! You should have 4 application attachments above...if any do not open, please let me know **Here's the website for one of the applications listed above: http://www.eatohillss.eq.edu.au/Library/About/Assistants/assistants.htm **Instead of or in addition to having teacher sign approval, you could have a parent sign ** "I have informed prospective helpers that if they act up during their class library time then I will not even consider them & if they come in during their "help" time & don't do what needs to be done then they will automatically no longer be a helper." ** "It might be better to get feedback from the applicants' last year's teachers as their new teachers may not know them well so early in the school year." ** "In the elementary school, I had a pizza party at the end of the year for my helpers. At the high school level, the students use the hours for community service. It's important to set up a system to document how many hours each person is working. At my high school, I have a three ring binder called "Student volunteers." Each student has a form that goes inside and is filed alphabetically. When a student comes in to work, he/she signs in with the date, time, and what task he/she worked on that day. At the end of the semester, we add up the hours and I fill out a form that goes to the guidance department to document that student has earned a set number of hours." ** "I have a K-5 building and only 4 & 5 graders can be helpers. With the emphasis on writing in our building, our children must write a short essay about why they want to be a library helper and why they think they can be successful." ** "Our guidance counselor does this for his ambassador program and one of the great questions on his application, or requirement I guess, is that the student must elicit one teacher recommendation.
