[LM_NET] A weeding form that you might find useful

I am in the process of weeding.  I have a collection of a little over
8000 titles.  During the last 2 weeks of school I generally have time to
go through the entire collection, looking for books to weed and books
that need repair etc.  I have developed a form that I find to be quite
helpful in the process.  If you are interested, please feel free to use
