When I took my children's lit class--here was the information needed

Title
Author
Publisher and year
Grade Level interest
Grade Level reading level (Decide how you will record: AR, Guided Reading, 
etc)
Artist
Art medium
Summary
Reaction to book
Genre/Nonfiction topic

I did mine on 5x8 cards color coded to the genre
Green:  Picture Book/Wordless
Blue: Historical Fiction
Yellow: Fantasy
White: Nonfiction
Pink: Animal
Purple: Media--Video, etc
Orange: Folktale
Lilac: Science Fiction
Aqua: Realistic Fiction
Gold: Sports Fiction
Neon Cherry: Other
