The only way I have to tried to solve some of this - and it happens more
than we want to think - is to gently take the item out of the teacher's
hands and tell them "I will sign it out for you -- helps me to know where
everything is." This does work. If I can't do that, then I make a written
note to myself that teacher A has item B - and I sign it out after they s/he
leaves.

I have been bold enough to walk to a teacher's room and gently say to the
teacher - 'I was so busy/distracted when you were in the library that I
didn't have time to sign this out to you. Let me do it now." Usually only
happens once and s/he know I am aware.

And in the big scheme of things - I have to ask myself if it is worth it to
embarrass the teacher and lose the contact - or is better to lose the item,.
But then, I have even gone to a teacher's  room after school and removed the
"stolen" item and returned it.
