Hi all,
        I'm toying with the idea of allowing students to check out books over our 
summer break. I work in a building with a K-5 population of about 750. We're in a 
pretty stable area--not too many kids "disappear" over the summer. Our summer break 
is only six weeks--we're on a modified year round schedule. Currently, students 
keep books during our quarterly breaks (about 3 weeks each) and loss is no more 
than it is during the rest of the year.
        If you allow students to take out books for the summer, I'd appreciate some 
advice. I'm trying to work out logistics before I make a final decision. I will 
only be checking out to K-4 since 5th graders will move on to middle school. Things 
I'm wondering about are:

        1. Do you require parents to fill out a permission form for their child to 
check out? (if you do, would you mind sharing the form?)
        2. How many books can individual students check out?
        3. Do you have a day(s) during the summer when they can return and get new 
books?
        4. Other considerations/things I'm forgetting?
