[LM_NET] TAR: permission slips for technology

Good afternoon friends,

My son's elementary school secretary just called, and they've purchased some ipods 
(I really think she means Playaways from Follett), and the principal would like 
some sort of permission slip/information sheet for parents.  

I searched the archives some, but I didn't find what I think she wants.  If any of 
you have something like this you'd be willing to share via an attachment, I'd be 
ever so grateful.
