[LM_NET] TARGET: Survey

I would like to give my faculty a survey at the beginning of the school year.  My 
goal is to get input from the staff as to the strengths of the media center and 
areas of services offered that the teachers may be unfamilar with or that they see 
as needing further attention.    I would also like to know of their experiences 
with school libraries, and what they liked and didn't like.  
 
Can you share your surveys?  I am interested in surveys appropriate for both 
elementary and middle school levels.  Thanks.
