[LM_NET] Target: MS or HS student aide applications

I have looked in the archives and found an application from 2000 and
wondered if anyone had anything newer or different, especially for
middle or high school.   What kind of questions do you ask?  Questions I
have already thought of:  Number of days absent in the fall semester. 
Number days absent this semester.  Are you responsible? Tell about an
instant of being responsible.   What else would you ask?  Thanks, Rita
