I had 33 responses to my question.  Of those, 16 said they didn't or couldn't 
require the teachers to pay the cost.  Of those 16, 7 expressed a wish that they 
could require payment, particularly due to 1 or 2 teachers who really abused the 
privilege.   10 said they required teachers to pay, sometime by official policy 
some times by unofficial. 1 of these had official policy to charge, but unofficial 
to let it go.  2 indicated they required payment unless there were special 
circumstances.
 
Paraphrasing a few comments:
Some didn't require payment because they felt it was better PR.
Some did require because it holds the teachers to the same standards as the 
students.
A couple of Texas folks mentioned there might be a bill in the legislature stating 
teachers couldn't be held financially responsible.
Some didn't require payment unless there was a habitual offender or large offense ( 
that wasn't resolved between librarian and teacher), then they talked to principal 
about the situation.
