Many thanks to all those responding to my request for application
questions for student assistants! I received several examples as
attachments-- please let me know if you would like me to forward them on
to you! 
 
Here is my original request with replies below:
 
I am developing an application for fifth graders to be assistants in the
media center. Aside from the obvious questions 

(Why do you want to be an assistant in the media center?...What
qualities do you have that would make you a good media center
assistant?), does anyone have other suggestions for questions? I
appreciate the assistance and will post a HIT.

I just had any student that was interested write a paragraph to express
their interest. They included why they would make a good media center
aide, and the time and days of the week they are available. I wish I had
the time to create an application but the start of the school year was
too busy! Good luck and I'd love for you to post your final application!

I spell out these expectations:

Do any job I ask, when I ask it.

Voices low.

Respectful of people and materials.

Be dependable, on time, and let me know if you can't come unless you are
sick.

I ask for work experience. Making beds, keeping room clean count. I'm
looking for responsibility.

I do an interview and go over the kinds of things they might need to do
and all of it isn't fun. And make sure they understand the expectations.

I have a place where I put that they will receive one warning at any
time and the second time at any time they will be asked to leave and not
return.

Child, parent, teacher reommending all sign

I've only had to dismiss one very noisy, not doing the job I asked, etc.
student in four years.

Of course, I fudge on it a bit, a warning this time and maybe another on
another day but shouldn't need to every time nor multiple times in one
session. My warnings are clear warnings and stated as such. Usually
pulls them back into line. I'm missing my 4th grade helpers and hope to
get some started soon. Even my volunteers are asking where they are!
They took care of most of our fiction shelving!
