[LM_NET] GEN: Equipment Permission form needed

Hello everyone!

For my mass media class students will be able to sign out some handheld mini-DV 
cameras to cover school activities. My principal would like for the parents to sign 
off on a form that says they will be responsible for the replacement cost if the 
equipment is damaged. Does anyone out there already have a form I can modify for 
this purpose. If you could email it to me today, I would really appreciate it!
