Dear Cindy
Perhaps you could add "My favourite TV show/movie is ..." so that if you have some 
TV/movie tie-ins
this would provide instant success .  

I would also keep the sheets in a folder so that the students can keep a reading 
record of the
subjects, authors and genres they have read so they can see their own progress. 
Nothing more than an
indicator that she read a book about dragons and next on the list is princesses.
If she returns a book by an author that she has enjoyed, say something like 
"Remember where we found
that, well you will also find others by XYZ there too."  
It's all about making them independent so have some private time with the student 
to get her
competent and confident in using the OPAC, despite her dyslexia. When it's 
appropriate, team her up
with a kindergarten kid so she can teach what she has learned. Show her you have 
faith in her
ability to succeed.
Keep her interests in the back of your mind and if you find something she would 
like, put it aside
for her and pre-empt her requests.  If you can build up a relationship with her 
through these little
things she might forget to be shy.

I think there a lot of kids who slip through the cracks because they are 
overwhelmed by the library,
so would love to see what others suggest.
