Does anyone have a permission form for Playaways they'd be willing to share? In talking to my principal, we decided to hold the Playaway for the student and have the student take a permission form home for the parents to sign. The next day, the child will be able to check out the Playaway. I've seen requests for comments about whether permission forms are needed, but I haven't seen requests for sample permission forms.
