[LM_NET] TAR: Birthday Book Club Book Plates?

New librarian here.  They used to type the information about the student on
these birthday book club name plates that are small and only fit in a type
writer.  We are looking for new name plates on a sheet page that we could
load into the printer and print them out on.  Where do you get yours at?
