[LM_NET] TAR: MS & HS - Reading Interest Survey Resources

Season's Greetings to all!

Our Reading Strategies instructor has asked if I know of an online or printed 
interest survey that she could use with her students.  She likes the format of this 
one from Book Adventure [ http://www.bookadventure.org/ki/bs/ki_bs_helpfind.asp ] 
but it's for K - 8 and looks very elementary in design.  Her students may read 
below grade level but do not want to be treated like elem. students.  Does anyone 
know a website that would work for her students?  I'm also working with her to 
create a printed one we could use but I'd prefer to go paperless if possible.
