[LM_NET] ELEM: Book Check out Policies

Hello All,
 
I am interested to know all of your book checkout policies.  I am seeking
primarily Elementary and Middle School.  But if other desire to respond that
is okay as well.  I will post a hit if desired.  It has been suggested that
I have a policy that is too strict. 
 
Questions:
How many for each grade levels?  
If they are not returned can they get an additional title?  
If lost what are the penalties?
