[LM_NET] Reading Survey for Elementary School

I am interested in developing a tool to evaluate student engagement in reading.  Do 
you know of some good student surveys for elementary students that measure 
motivation, interest, quality book choice? My students are mostly English as a 
second language, so I am looking for something simple but clear that involves some 
kind of point system.  Anything effective out there that you can share for me to
