Hello Colleagues,

Your Help is needed! I am seeking your opinions for my research project about 
professional library associations. I am a school librarian and currently a doctoral 
student at the University of Wisconsin - Milwaukee. I am researching perceptions of 
professional library associations and association membership from school 
librarians. Please consider taking a few minutes to complete my professional

Consent to Participate in Online Research

Study Title: “Leaders and Links: Perceptions of Professional Association 
Membership by School Library Media Specialists”
Person Responsible for Research:  PI: Dr. Elizabeth Buchanan     Co-PI: Janet Amann
        
Study Description:  The purpose of this research study is to delve into perceptions 
of professional library / media association membership as expressed by K-12 school 
library practitioners. Approximately 300 subjects will participate in this study.  
If you agree to participate, you will be asked to complete an anonymous electronic 
survey that will take approximately 10 – 15 minutes to complete.  The electronic 
survey will ask general demographic questions, and collect thoughts and opinions 
about professional library association membership and the roles various types of 
associations play in meeting current and future needs and challenges for the field 
of school librarianship.

Risks / Benefits:  Risks to participants are considered minimal.  There will be no 
costs for participating, nor will you benefit from participating other than to 
further research about professional library associations. Although there are no 
completely secure interactions online, the survey is anonymous. Security procedures 
are in place for data collection and against intrusion, but as an online 
participant in this research, there is always the risk of intrusion by outside 
agents, i.e., hacking, and therefore the remote possibility of being identified. 
The results of the survey might be used in a written document or presentation, but 
your answers are completely anonymous. The initial data collection with any 
possible identifiers and data analysis are stored on two different servers.

Confidentiality:  Your responses are completely confidential and no individual 
participant will ever be identified with his/her answers.  Names or IP Addresses 
are not collected in the survey. Data from this study will be saved on a password 
protected computer for two years and then destroyed.  Only PI Dr. Elizabeth 
Buchanan; Co-PI Janet Amann; Doctoral Committee members Dr. Raji Swaminathan and 
Dr. Dietmar Wolfram; and the graduate technical research assistant will have access 
to the data. Results of the study may be summarized and presented or used as part 
of research publication, but no information will be included which could identify a 
subject.

Voluntary Participation:  Your participation in this study is voluntary. Completing 
the survey directions and signifying consent indicates that you have read this 
consent form and have had all of your questions answered, and that you are at least 
21 years of age. You may choose to not answer any of the questions or withdraw from 
this study at any time without penalty. Your decision will not change any present 
or future relationship with the UW – Milwaukee or the organization list-serve.
