[LM_NET] TAR Book Challenge Letters

Dear LM Netters,
 
Has anyone ever served on a Challenged Book review committee?  If so, did you help 
to write a formal letter to the Superintendent (or other administrator) stating the 
committee's decision whether the book would be removed or not?
 
I am interested in how these letters are written and what types of information you 
included.  Also, did you include a template in your school board policy manual?
