TARGET: SEC. Library Surveys

Good Morning,  I have been asked by admin. to create a survey for =
teachers as well as students for evaluation of Library Media Center =
services.  This will answer reccomendation by accreditation committee =
and provide valuable input for me (this is my 3rd year in the library =
and I'd love some feedback).  I checked the archives and got a few =
ideas, some older than others.  I wondered if anyone out there had =
something they are very happy with and is fairly quick and easy for =
patrons to do.  Any help would be , as usual, much appreciated.  Thanks =
