policies for videotaping students

I am looking for policies and permission forms that schools use to
obtain permission from parents before videotaping students in school.
The intent is to protect the school if a recording is released to a
larger audience outside the school.  If your school or district has
such a policy and/or permission form, please send me a copy. Snail mail
is fine, this is not urgent.  Thank you.
