LM_NET: Library Media Networking

I am looking for a program to develop and fill out forms on the computer.  I
presently use Smart Form Designer & Assistant from Claris, but Claris sold off
the software several years ago.  Calling Claris would probably yield the name
of the company, but I would appreciate a recommend from The Net.

If state departments of education could stop killing trees and use our
electronic boxes as the next generation tool, not just nice typewriters.
Although I must give our DPI credit, I have seen disks with blank forms done
with  a word processor sent out to some departments.  Think there could be
another interesting survey here.

Does anyone use a use this type of software, if so please answer the following:

        1. Software name & publisher
        2. Cost
        3. Do you use it and if so does it work well
        4. Can you use a scanner to digitize forms and input into the software
        5. Macintosh, DOS, or Windows (I would prefer to use the Mac, but we
can use other OS's.)
