Forms needed

Help!  I am taking a School Library Management course, and the

professor wants us to bring library evaluation forms.  No such

animal seems to exist in our system.  Does anyone have a form

that would fit the bill?  He also wants forms that give job

descriptions for media specialists, clerks, student helpers,

and volunteers.  We can't seem to find any of those,either!  If

anyone can either mail, fax, or e-mail the forms, I'd really

appreciate it.

ps.  He also seems to think that everyone has a policy manual

for their school.  I've asked around in my system, and nobody

seems to have one.  Do you find this to be true in your systems?
